The supine head-down tilt position that was named after the German surgeon

چکیده
Background: The supine head-down tilt position is routinely used in intensive care when inserting a central venous catheter to increase the lumen of the jugular or subclavian vein and to prevent air embolism during this procedure or to correct severe hypotension. The head-down tilt position is credited to the German surgeon Friedrich Trendelenburg (1844–1924), and is consequently named after him as the ‘Trendelenburg position’ or ‘Trendelenburg posture’. Methods: Analysis of relevant literature. Results: Although a brilliant surgeon and excellent scientist, Friedrich Trendelenburg was not the first who utilized the posture that is named after him. The Dutch physician Abraham Cyprianus used a similar technique more than 170 years earlier in 1694. Furthermore, the terms ‘head-down tilt posture’ or ‘semi-inverted posture’ are more accurate to describe the posture in which we place our patients nowadays. This posture is not the same posture as the classic ‘Trendelenburg position’. The posture in which we place patients during diagnostic, therapeutic and experimental interventions nowadays is identical to the ‘semi-inverted posture’ described by Cyprianus in 1694 and by Hewitt & Sheild in 1896 and not to Trendelenburg’s posture. Conclusions: The posture in whichwe place patients during diagnostic, therapeutic and interventions and during clinical experimental research nowadays is identical to the ‘semi-inverted posture’ described by Hewitt & Sheild in 1896 and not to Trendelenburg’s posture. We should name it in the most accurate way (semi-inverted posture).

Thermoregulatory response to intraoperative head-down tilt.
UNLABELLED Thermoregulation interacts with cardiovascular regulation within the central nervous system. We therefore evaluated the effects of head-down tilt on intraoperative thermal and cardiovascular regulation. Thirty-two patients undergoing lower-abdominal surgery were randomly assigned to the 1) supine, 2) 15 degrees -20 degrees head-down tilt, 3) leg-up, or 4) combination of leg-up and he...
متن کاملEffects of prolonged head-down tilt on internal jugular vein cross-sectional area.
BACKGROUND Head-down tilt is often used to assist cannulation of the internal jugular vein (IJV). However, the optimal duration of tilt before cannulation is not well defined. METHODS Fifteen healthy volunteers were studied by B-mode duplex sonography to assess changes in cross-sectional area (CSA) of the right IJV during 10 degrees head-down tilt. RESULTS Median CSA in the supine position ...
متن کاملThe influence of body position on Bielschowsky's test.
PURPOSE To investigate the veracity of Jampolsky's statement that Bielschowsky's head tilt test is inverted if performed with the patient in the upside-down position and to interpret its neuromuscular mechanism. METHODS We present a series of 10 patients selected from a referred sample who were diagnosed with superior oblique paresis. Hypertropia was measured in the primary position, with the...

Sir,—In commenting on our paper [1], Kardos, Rudas and Simon described their own use of the analysis of short-term heart rate variability to assess the effect of varying posture on cardiac autonomic control. Essentially they have confirmed that head-up tilt causes sympathetic activation (as evidenced by an increase in the low frequency variability) in comparison with supine and head-down positi...
متن کاملThe Trendelenburg position: a review of current slants about head down tilt.
The steep head down tilt surgical posture, popularized in the 1870s by Trendelenburg as a means of improving access to pelvic pathology and espoused by the American physiologist, Walter Cannon, during World War I as a resuscitative position with which to treat shock, has a history of widespread, ritualistic acceptance. An awake patient placed in steep head down tilt usually objects to the postu...
